TAC Injury Support Services
Supporting people recovering from transport accident injuries — funded through the Transport Accident Commission (TAC) in Victoria
TAC Injury Support Services
Positive Care & Supports can provide disability and community support services for people recovering from transport accident injuries funded through the Transport Accident Commission (TAC).
Our team supports people living with physical, neurological, and psychosocial injuries following road accidents, helping them rebuild independence, community participation, and daily living skills.
We work collaboratively with TAC case managers, hospitals, rehabilitation providers, and families to ensure supports are practical, person-centred, and focused on long-term recovery and independence.
What's Included:
- Attendant Care — personal care, hygiene support, meal preparation, and household tasks for people with acquired injuries
- Community Access & Participation — supported engagement in social, recreational, and community activities to rebuild connection and independence
- Independent Living Skills — structured programs to help clients regain everyday skills and manage their home and daily routine
- Supported Accommodation — support to find and maintain appropriate housing during and after recovery
- Behaviour & Therapeutic Support — evidence-based strategies to address behavioural and psychosocial challenges arising from acquired injury
- Case Coordination & Recovery Planning — coordination across services and providers to ensure a clear, consistent recovery pathway
Who We Work With
Our team is experienced in complex disability support and works collaboratively with:
- TAC case managers — coordinating supports aligned with the client's TAC-approved funding and recovery goals
- Hospitals and rehabilitation teams — supporting safe and sustainable discharge planning from acute and sub-acute settings
- Legal representatives — providing documented support services relevant to TAC claims and long-term care needs
- Families and carers — working alongside families to ensure continuity of care and appropriate support in the home
- Allied health providers — collaborating with physiotherapists, occupational therapists, psychologists, and speech pathologists as part of a coordinated recovery team
How the Process Works
TAC clients do not need to be registered with us in advance. Services are arranged through a referral from your TAC case manager, Support Coordinator, or treating team.
Referral from TAC
Services are arranged through TAC. Your Support Coordinator or treating team will assess your needs and, if appropriate, refer you for support.
Understanding Your Needs
A qualified health professional, often an occupational therapist, will complete an assessment to understand what support will best assist your recovery and daily living.
Your Personalised Support Plan
Once approved, we work with you and your care team to develop a tailored plan outlining the type of support you'll receive, how often it's provided, and the goals you're working towards.
Support Begins
Our support workers start providing care in line with your plan, assisting with everyday tasks, rehabilitation-focused support, and building confidence as you move forward.
